Our Community Transition Beds support patients during times of increased demand, offering short-term, post-acute recovery and transitional care. As a Licensed Practical Nurse (LPN), you will play a key role in delivering safe, competent, and compassionate nursing care that helps individuals regain independence and quality of life after illness, surgery, or hospitalization. In this role, you will perform clinical assessments, administer medications, monitor patient progress, and collaborate closely with Registered Nurses, Health Care Aides, and other members of the multidisciplinary team. Your ability to combine technical skills with empathy ensures holistic care that addresses physical, emotional, and spiritual needs. Description: The Licensed Practical Nurse (LPN) works as a member of the multidisciplinary health care team, collaborating on the delivery of holistic, comprehensive care and support to patients/residents and families. The LPN must be able to team lead effectively on their own. The LPN works to the full scope of practice in providing safe, quality care in accordance with established standards, and the Mission, Vision, Values, policies and procedures of Covenant Health. This position is covered by the terms and conditions of the AUPE Auxiliary Nursing Collective Agreement.

Required Qualifications: Graduate of a Practical Nursing program or recognized equivalent required. Active registration with the College of Licensed Practical Nurses of Alberta (CLPNA) required. Basic Life Support Healthcare Provider (Level C) required.
Additional Required Qualifications:Minimum of 1 year of experience in a long-term care or geriatric setting. Basic knowledge of long-term care principles, including chronic disease management, medication administration, and wound care. Strong clinical skills and the ability to perform nursing tasks with accuracy and compassion. Excellent communication skills, with the ability to work effectively with residents, families, and the healthcare team.Ability to demonstrate empathy, patience, and professionalism in all interactions. Physically able to complete all demanding aspects of patient/resident care and transport, e.g. walking, standing, bending, lifting, assuming awkward positions and standing for long durations of time. Must be able to understand written and verbal directions in English.
Preferred Qualifications:Certification or additional training in Geriatric Nursing or Long-Term Care Nursing (e.g., Certified Nurse in Long-Term Care (CNLTC)). Certification or training in crisis management techniques, such as Nonviolent Crisis Intervention (NCI) or Therapeutic Crisis Intervention (TCI). Experience with electronic health record (EHR) systems, such as Connect Care. Experience with InterRAI MDS/ LTCF Assessments.
